Copy & paste this info into the appropriate fields when submitting your complaint. This makes sure you have a copy of your complaint after you submit it (The 311 system won't give you a copy of what you submit).
83 | * Write what you were doing (biking, walking, etc) to help job your memory about your relation to the incident.
